Money follows the person, says Care Connection

Options Counseling and Transition Coordination Services are available for elderly and disabled individuals who wish to reside in their community. This service is funded through the Money Follows the Person (MFP) Demonstration Grant which was awarded to the Missouri Department of Social Services (DSS) in January 2007 by the Centers for Medicaid and Medicare Services.

Through this grant, DSS has collaborated with the Division of Senior and Disability Services (DSDS) to offer assistance to individuals as they transition from a nursing home to a community setting. In February of 2012, DSDS offered contracts to the Centers for Independent Living and Area Agencies on Aging to provide these services. Care Connection for Aging Services is excited to be participating in this innovative transition opportunity.

Individuals who are eligible for Transition Coordination Services through the MFP Demonstration Grant receive case management services, to include monthly face to face visits throughout the year following the transition to a community setting. In addition, funding can be provided for those individuals who may need assistance to establish a residence in the community, which may include rental and utility deposits.
